Make the Telegram webhook work on a single Worker

Previously required a second Worker unconditionally, even for a
Telegram-only deployment with no Discord at all. The split into
telegram-worker.ts only earns its keep when running both platforms
and specifically wanting them bundle-isolated from each other -- so
the shared handler now lives in adapters/telegram/webhook.ts and gets
called from both src/worker.ts (default, single Worker) and
src/telegram-worker.ts (optional, for the bundle-isolation case).

Adds docs/telegram-adapter.md and updates the README accordingly.
