Tighten the README and docs, fix the setup steps that went stale

deploy-cloudflare.md and configuration.md still told people to put
DISPOSABLE_DOMAIN in wrangler.jsonc under vars, which stopped being
true when it moved to secrets. Both now describe the secrets flow and
say why: the file is committed, and plaintext dashboard variables get
overwritten by it on the next deploy.

Otherwise this is a prose pass. Cut the hedging and the
"worth knowing" asides, shortened the walkthroughs, moved Expiry
above Notes in the Discord guide since expiry is the thing people
hit first. Same information, about 700 fewer words.
