Preserve link URLs in HTML-to-text conversion

Buttons/links were rendering as bare text with the href silently discarded,
which matters for action emails (sign-in confirmations, password resets)
where the destination is the whole point. Now renders as "label (url)" and
drops empty/fragment/javascript: hrefs instead of surfacing dead links.
