Make the Telegram and Slack adapter docs self-contained

Both leaned on discord-adapter.md for command semantics, rate limits,
and expiry/note behavior instead of stating their own -- fine for
someone reading all three in order, not fine for someone who only
ever needed Telegram or only ever needed Slack. Each guide now has
its own full command table and expiry/notes/reminders sections,
written in that platform's own argument syntax.
