Cut redundant Discord mentions from README down to what's load-bearing

"Discord" was showing up 6+ times as connective tissue (opening line, how
it works, limits, a whole DM/server/channel paragraph duplicating what's
already in docs/discord-adapter.md) even after already moving the
Commands table out. Down to 4 mentions, each doing distinct necessary
work: declaring it once as the current delivery target, the link to its
setup guide, and two architecture notes about what specific files do.
Everything else now says "delivered" / "you" instead of restating the
adapter name every time delivery comes up.
