Bump to v0.6.0, fix README staleness from the Slack adapter

- package.json version and description (still said "delivered to
  Discord" only).
- README's "Under consideration" list still had Slack in it after it
  shipped.
- Limits section stated Discord's inline-body and attachment caps as
  if universal; Telegram and Slack use different numbers (3500 chars,
  50MB per file vs. Discord's 1500 chars, 25MB combined).
