Remove the migration scaffolding: D1 import path, one-shot cron steps, backfill helpers and unused exports

The scoped schema generator now reads the migrations up to the drop; new scoped DDL goes in schema-extra.ts. Tests that model every store in one sqlite use applyCombinedSchema.
