Check attachment quotas against per-account totals instead of every repository

Every upload summed attachment sizes across all repositories. Each account now keeps its owned and uploaded bytes per repository, recomputed from that repository's rows on upload, sweep and deletion, so a retry or the cron backfill can't double-count. Until the backfill finishes, the old sum stays.
