Extract inline colors and UI primitives into a design system under src/ui

Move Button, Field and the shared primitives out of src/components/ui into src/ui,
add Avatar, Badge, Callout, Code, CountBadge, EmptyState, Heading, LabelChip,
Markdown, Nav, PageContainer, Panel, ProgressTrack, RelativeTime, StatusDot,
TextLink and a shared tone palette, and switch components and routes to them.
