Add a public created/torched counter page (Cloudflare only)

GET / on the Worker serves a small themed page, GET /counters backs
it with JSON. Counts come from a running counters table rather than
COUNT(*) on addresses, since deleteExpiredAndRevoked physically
deletes rows during daily cleanup and a live count would lose
history the moment that runs. incrementCreatedCounter/
incrementTorchedCounter only fire on an actual insert/revoke
(result.changes > 0), so retries and no-op torches don't inflate
the numbers.

Self-host and mail.tm untouched, this only wires into src/worker.ts.
