Add permanent addresses, good until torched

A `permanent` flag on the addresses table, plus an optional
permanent:true/false on /new and /extend to set or clear it. Kept as
a flag rather than a null expires_at so clearing it later falls back
to a fresh expiry instead of one that lapsed in the meantime.

Every "is this active" query and the inbound lookup now treat a
permanent row as unexpired, and cleanup's expiry branch skips them
entirely, so /torch stays the only thing that ends one.
